In this position, you will meet one-on-one with patients to ensure they comprehend their care options.Key Responsibilities:Facilitate status-neutral engagement activities that promote access to HIV biomedical interventions for prevention and care.Deliver direct care to individuals seeking services, including conducting intake assessments for HIV care, PrEP/PEP, and gender-affirming care, providing sexual health education, and assisting patients with insurance navigation.Identify and address psychosocial barriers to care, facilitating connections to supportive services and relevant programs.Monitor patient access, adherence, and retention in care, while maintaining comprehensive knowledge of internal and external resources to ensure a seamless continuum of care.Collect and analyze data to track program progress and apply quality improvement methodologies to enhance operations.Act as a liaison between local agencies and the program's operations.Interpret and communicate program guidelines, policies, and procedures effectively.Engage in quality improvement initiatives by contributing to program innovation and the establishment of operational goals.Uphold confidentiality and ensure HIPAA compliance according to clinic policies.Perform other related duties as required.
