Primary Purpose:

We take pride in the exceptional care provided to our members, and as our business continues to grow, we are seeking a dedicated and skilled Nurse Practitioner to expand our Advantmed provider network.

In this vital role, you will perform in-home wellness risk adjustment assessments for Medicare beneficiaries and other at-risk populations. Your expertise will help assess their overall health and well-being, ensuring precise and comprehensive risk adjustment coding that enhances value-based care. This is a 1099 (PRN) position, offering significant flexibility for those juggling various commitments.

Key Responsibilities:

Conduct in-home clinical assessments, including:

  • Traveling to members’ residences to perform comprehensive health evaluations
  • Executing Annual Wellness Visits and assessments for members with chronic conditions
  • Conducting medical history reviews, physical examinations, and vital signs collection
  • Reviewing and reconciling medications
  • Performing preventive health screenings such as Hemoglobin A1C, Spirometry, Kidney Health Evaluations, and Diabetic Retinal Exams

Engage with patients through education and support to foster their health and wellness.
