Primary Purpose:

Our commitment to delivering exceptional care to our members is unwavering, and as our team grows, we are seeking a dedicated and skilled certified Nurse Practitioner to join our network at Advantmed.

 

In this role, you will conduct in-home wellness risk adjustment assessments for Medicare beneficiaries and other at-risk populations. Your primary goal will be to evaluate the overall health and well-being of our beneficiaries, ensuring precise and comprehensive risk adjustment coding that promotes value-based care. This position offers flexibility as a 1099 (PRN) role, ideal for those managing various commitments.

NP Responsibilities:

In-home Clinical Assessments:

  • Travel to members’ residences to perform thorough health evaluations.
  • Conduct Annual Wellness Visits and comprehensive health assessments for members with chronic conditions.
  • Perform essential evaluations, including:
  • Medical history and physical examinations.
  • Systems review.
  • Collection of vital signs.
  • Medication review and reconciliation.
  • Preventive health screenings as necessary:
  • Hemoglobin A1C.
  • Spirometry.
  • KED (Kidney Health Evaluation).
  • DRE (Diabetic Retinal Exam).
  • FIT.

 

Patient Education & Engagement:

  • Deliver tailored health education to empower our members in their health journeys.
