- Evaluate and monitor patient conditions through vital sign assessment; accurately record intake and output; maintain patient care journals while keeping family members informed.
- Administer necessary treatments including medication dispensing, bandage changes, oxygen administration, and upkeep of equipment.
- Document patient information meticulously in care journals; promptly notify nursing supervisors of any changes or unusual conditions.
- Assist patients in self-care by teaching assisted living techniques including the use of canes, walkers, and special utensils for eating, as well as providing guidance on personal hygiene.
- Support family members in patient care by instructing them on safe lifting and repositioning techniques, connecting them with additional resources for home care, and guiding them through Medicare reimbursement processes.
- Ensure a safe and healthy environment for patients by adhering to aseptic standards, changing linens, maintaining cleanliness in kitchens and bathrooms, and managing personal laundry.
- Prepare and serve nutritious meals according to prescribed dietary requirements and nutritional standards.
- Maintain continuity of care by documenting actions and needs in patient journals for the next caregiver.
- Enhance professional skills and maintain CNA certification through ongoing education and professional development opportunities.
- Uphold the integrity of the home health care organization through adherence to professional standards and compliance with all relevant regulations.
- Contribute to the positive reputation of the nursing assistant and home health care organization by taking ownership of responsibilities and seeking opportunities for improvement.
