Our RequirementsActive LCSW, LMFT, LPC, or LMHC license in New York. Must be fully licensed and capable of working independently. Strong foundation in evidence-based modalities. Comfortable meeting clients in community or residential settings. Proof of professional liability insurance ($1M/$3M coverage) or willingness to secure it prior to hire. Active NPI number. Must be able to pass a criminal background check. Reliable transportation and ability to travel locally to meet clients. Even if you don’t meet every qualification, we encourage you to apply. About This Opportunity:

We are expanding our in-person care team and are on the lookout for a Clinical Social Worker or Licensed Therapist to be the compassionate representative of Total Life within the community.

This distinctive role combines clinical practice with community engagement, allowing you to provide direct support to older adults through wellness assessments and initial therapy sessions at our partner residential locations, facilitating their connection to ongoing care.

You will embody the clinical face of Total Life in the field, effectively bridging the gap between older adults and the emotional support they rightfully deserve.

Primary Responsibilities:

  • Conduct initial clinical sessions and develop tailored recommendations for ongoing therapy or support.
  • Perform in-person wellness assessments for older adults to assess emotional, cognitive, and social well-being.
  • Act as a representative of Total Life in the community, serving as the local point of contact for clients, caregivers, and partners.
  • Collaborate with our virtual therapy team to facilitate seamless transitions into ongoing care.
  • Ensure accurate and timely clinical documentation.
  • Engage in team meetings and contribute to enhancing our community-based programs.
