Position Summary:
As a Case Manager in the Health Systems Navigation (HSN) team, you will deliver holistic, client-centered case management with a focus on harm reduction for individuals who use substances. Your role will be pivotal in ensuring seamless access to and sustained engagement in a comprehensive range of health services, which encompasses primary care, behavioral health, substance use treatment, and HIV/HCV/STI prevention and care. Key responsibilities include conducting outreach, testing, counseling, and facilitating support groups, particularly aimed at re-engaging participants who are newly diagnosed or have disengaged from care. By managing a dedicated caseload and crafting individualized care plans that prioritize goals identified by clients—such as enhancing health, securing stable housing, and navigating social services—you will play a critical role in supporting participants on their journey toward improved health, stability, and self-sufficiency.
