Qualifications:Bachelor’s degree in a relevant field such as child & family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreational therapy, rehabilitation, social work, sociology, or speech and hearing; ORNYS licensure and current registration as a Registered Nurse with a bachelor’s degree; ORA bachelor’s degree in any field with five years of experience directly working with individuals with behavioral health diagnoses; ORA Credentialed Alcoholism and Substance Abuse Counselor (CASAC). Additionally, candidates should have:A minimum of two years of experience (a Master’s degree in a related field may substitute for one year) in either:Providing direct services to individuals with serious mental illness, developmental disabilities, or alcohol/substance abuse; ORLinking individuals with serious mental illness, developmental disabilities, or alcohol/substance abuse to necessary services.

About the job

As a Care Manager, you will deliver patient-focused, high-quality care management services to clients with complex chronic conditions. Your role involves effectively navigating various systems, including primary medical, specialty, behavioral health care, and social services, ensuring timely support for our clients.

Key Responsibilities:

  • Deliver comprehensive care management services, including outreach, engagement, assessment/reassessment, and the development and implementation of individualized care plans. Collaborate with primary care physicians and other providers to create crisis intervention plans.
  • Coordinate care and promote health by working with service providers and health plans to secure necessary medical care. Share emergency information, link clients to services that support care goals, provide patient education, and advocate for services while assisting with scheduling.
  • Manage transitional care by following up on admission and discharge notices from ER, hospital, residential, or rehab settings. Facilitate discharge planning with relevant staff and ensure safe transitions to care.
  • Support clients and their families through care planning and education about healthcare needs. Organize meetings, provide referrals to support groups, and collaborate with community providers to enhance service utilization.
  • Link clients to community and social support services, ensuring access to medical, behavioral health, substance abuse, and other necessary services.
  • Complete timely and thorough documentation, including notes and assessments.
  • Adhere to company policies and corporate compliance standards, maintaining necessary certifications and licenses.
  • Participate in training, in-service sessions, and departmental meetings as required.
  • Perform additional duties as assigned by your supervisor or director.
