Job Summary
We are in search of a warm and empathetic Cardiology Nurse Practitioner or Physician Assistant to become a vital part of our healthcare team. In this role, you will provide comprehensive care to patients experiencing heart-related conditions, making a significant positive impact on their lives while working alongside a committed group of healthcare professionals.
Responsibilities
- Conduct thorough patient assessments, including physical exams, medical histories, and diagnostic tests.
- Partner with cardiologists to formulate and execute treatment plans for patients with cardiovascular issues.
- Administer medications, observe patient responses, and modify treatment plans as needed.
- Perform cardiac stress tests, electrocardiograms (EKGs), and other diagnostic evaluations.
- Instruct patients and their families on cardiovascular health, preventive strategies, and the importance of medication compliance.
- Offer counseling and emotional support to patients and their families.
- Familiarity with CMS Regulations and NCQA HEDIS Guidelines.
- Maintain accurate medical documentation to ensure adherence to OSHA and governmental regulations while performing duties in accordance with site quality and safety standards.
- Collaborate with the interdisciplinary team to optimize patient care.
- Assess and coordinate the care of critically complex patients.
Requirements
- A minimum of 6 months of experience in long-term care or a skilled nursing facility.
